Welcome to Our Loyalty Program!

The process is simple. You earn points for every visit to the office. You can earn points on anything in the office including exams, grooming, boarding, or even pet food and medications. All you have to do is present this card to reception at checkout to get your points added. If you forget to present your card, you can also call in and provide your unique card code (found under your QR code). As you earn enough points, you’ll see your rewards automatically unlock for you to then redeem at your next visit. We’re thrilled to have you as a loyal member of our practice…it means the world to us! Happy earning!

Terms and Conditions

Welcome to our Loyalty Program (“Program”), operated by Business Growth Elite Inc, 658 N. Milwaukee Ave. #100-2604, Chicago, IL 60647, DBA “UPbook” (“Company,” “we,” “us,” or “our”). Participation in the Program is subject to the following terms and conditions. By enrolling in or using the Program, you agree to be bound by these terms and conditions, which form a legally binding agreement between you and Company.

Loyalty membership eligibility

Membership in the Program is available to any United States resident who is at least eighteen (18) years old and provides valid and accurate personal information when enrolling. Employees of the Company and their immediate family members are not eligible to participate.

Participation in a separate and unique credit or debit card program is not required for a general membership. Once enrolled into the Program, you will enter at the member status level and are entitled to the rewards and benefits of membership as set out herein or in promotional materials.

Points Accumulation and Redemption

Participants in the Program are eligible to accumulate points (“Points”) based on qualifying activities as detailed in the Program guidelines. These Points are attributed to the account of the participating member (“Participant”) upon completion of such activities. Points accumulated under the Program are for the Participant’s use only and represent the Participant’s standing within the Program.
Participants acknowledge that Points are not personal property, have no cash value, and cannot be used as a medium of exchange for value from any other individual or entity. The sole method for utilizing Points is in accordance with the redemption options provided by the Company within the Program structure.

Non-Transferability of Points

Points earned in the Program are strictly non-transferable. Points cannot be sold, traded, bartered, assigned, or transferred to another person, including but not limited to any other Program participant, either directly or indirectly, by operation of law or otherwise. This prohibition includes the transfer of Points upon death, as part of a domestic relations matter, or in any other transfer to a third party. Any attempt to sell, transfer, or assign Points in violation of these terms will lead to immediate forfeiture of said Points and may result in the termination of the Participant’s account in the Program.

Membership conditions

Participants must maintain an active Program membership to earn and redeem Points. You may opt out of the Program at any time. If you opt out of the Program, you will no longer earn Points or be able to redeem Points and you will forfeit your unused and unexpired Points. Deletion of Points is final and cannot be reversed. To cancel a Program membership, please call Customer Service.

Expiration dates

Points have no monetary value and they will not expire if you make at least one purchase every 12 months. Points expire after 12 months if you do not make any purchase in the 12 months after earning them.

Eligible products or services

Please note that Points cannot be earned for: any purchase made using a reward coupon; store credit or merchandise credit; charity donations; purchase of a gift card; or the value of any gifts-with-purchase. All other purchases will be qualifying purchases.

Purchase requirements

A minimum merchandise purchase of $10.01 (excluding taxes and shipping charges) is required in order to redeem Reward coupons.

Promotion stacking

Participants can only use one coupon or voucher at any one time when making a purchase, and it cannot be combined with any other coupons, vouchers or discount codes or be applied to discounted items. The coupon or voucher will only apply to the product price and will not apply to any delivery costs.

Redemption conditions

Only Participants in good standing with active accounts are eligible to redeem Rewards. An account must not be suspended, canceled, or otherwise inoperative due to breach of the Program terms and conditions or for any other reason. The Rewards available for redemption and the number of Points required for such redemption are subject to change at Company sole discretion without prior notice. Information about available Rewards and required Points for redemption can be found on the Program’s website or upon request from the Program’s customer service.
To redeem Points for Rewards, members must follow the redemption process as outlined by the Company. This process may require members to verify their account information, select the desired Reward, and confirm the redemption transaction. Upon successful redemption, the corresponding number of Points will be deducted from the member’s account. Redeemed Rewards are subject to the terms and conditions applicable to each specific Reward, including expiration dates, and restrictions on use
Once Points are redeemed for Rewards, the transaction cannot be reversed, and Points will not be re-credited to the member’s account upon cancellation or return of the redeemed Reward, except as solely determined by Company under exceptional circumstances.

Tiers specification

A Participant’s membership tier level is established based on the Points received within their current tier level. This determination is made by evaluating the number of Points a Participant has accumulated through various qualifying activities as per the loyalty program’s outlined criteria.
New Participants, or those who have not yet accumulated the necessary number of Points for advancement to the second membership tier level, will commence their participation in the loyalty program at the first tier. This foundational tier serves as the entry point into the loyalty program, offering a baseline of benefits and opportunities for Points accumulation.
Once a Participant has accumulated enough Points for the next tier, they will be presented with the option to upgrade their membership tier level. This advancement recognizes the Participant’s engagement and loyalty, unlocking additional benefits, privileges, and access exclusive to the higher tier. The option to upgrade is a milestone event in the Participant’s journey within the loyalty program, signifying their elevated status and commitment.

Program Modification or Termination

Company reserves the right to modify any of the terms and conditions of the Program, including the benefits and rewards offered, at any time without prior notice.
Company reserves the right to terminate the Program at any time. Upon termination, participants will have a specified timeframe to redeem any accumulated rewards.

Fraud prevention

Company reserves the right to cancel or suspend participation in the Program or any Rewards earned and not yet used in the event of fraud, abuse of rewards privileges, violation of these Program Terms and Conditions, or at Company discretion.

Limited liability

To the fullest extent permitted by applicable law, no responsibility or liability is assumed by the Company for technical problems or technical malfunction arising in connection with any of the following occurrences which may affect the operation of the Program and/or Site.

Personal data protection

You consent to the collection, use, and disclosure of your personal data by the Company, the Loyalty Program, Participating Properties and Partner Programs, and their authorized third-party agents and licensees in accordance with the Company’s Privacy Policy.

Dispute Resolution

Prior to initiating any formal dispute resolution proceedings, the parties agree to attempt in good faith to resolve any dispute arising out of or relating to the Program directly through negotiation with senior management of the Company. This informal negotiation period shall last no less than thirty (30) days from the date either party communicates to the other, in writing, a desire to negotiate under this section.
If the dispute cannot be resolved through informal negotiation within the timeframe specified above, then, the dispute shall be resolved by binding arbitration administered by the American Arbitration Association (“AAA”) in accordance with its Commercial Arbitration Rules

Governing Law

Except as otherwise provided for herein, this dispute resolution clause and the arbitration conducted pursuant to it shall be governed by the Federal Arbitration Act (9 U.S.C. §§ 1 et seq.)
